Suburban Care Collaborative CIN Overview

The Suburban Care Collaborative Clinically Integrated Network (CIN) is a physician-led collaboration between a committed group of Suburban Health Organization partner Hospitals who are working together and depend on one another to bring the best in care quality and care experience to the patients they serve, to reduce avoidable costs and care inefficiencies, and to optimize the healthcare delivery experience for their clinicians. These goals are to be achieved through dedicated physician leadership, a ‘hive mind’ approach to best practice identification, evidence-based guideline creation and investments in data analytics. Ultimately the Suburban Care Collaborative CIN is a performance engine to help all Suburban Health Organization partners best serve their communities, regardless of their direct CIN participation.

The CIN Participating Hospitals include:

Physicians and Advance Practice Practitioners from our Participating Hospitals actively participate in the Suburban Care Collaborative CIN by serving on key committees and by defining / refining the clinical and quality benchmarks that measure their performance.

 

Leadership:

The efforts of these committees are governed by the Suburban Care Collaborative CIN Board, populated predominantly by Physicians. Suburban Health Organization staff provide executive leadership support, regulatory compliance functions, comprehensive data analytics and facilitative support.

Our Ultimate Goal is to fulfill the Institute for Healthcare Improvement’s Quadruple Aim of

  2. Improving the health of populations
  3. Reducing the per capita cost of healthcare
  4. Improving Clinicians’ experience in providing care

We Work to Achieve These Goals By:

  • Utilizing meaningful quality measures chosen by Physicians to gain deeper insights into clinical care and to improve the clinical outcomes in the community
  • Coordinating and navigating care
  • Making care delivery as efficient as possible
  • Improving patient outcomes through the deployment of evidence-based clinical guidelines
  • Partnering with various insurance providers to drive clinical improvement but using ‘Payer agnostic’ tactics to achieve such improvement.

Operational areas of focus for the CIN include the following:

  • Efficiency of Care oversight – monitoring that care is received in the right setting at the right time by the right caregivers.

Suburban Health Organization regards the Suburban Care Collaborative CIN as a learning lab built upon Physician engagement and innovation. As such, we expect that all SHO partner Hospitals and their communities, regardless of their CIN membership, will benefit from CIN initiatives.
