As of 2023, six SHO Partner Hospitals have created a formal Clinically Integrated Network (CIN) structure, designed to accelerate and optimize care transformation efforts within value-based care.

Organized Health Care Arrangement Declaration

Suburban Care Collaborative Clinically Integrated Network ("CIN") is an Organized Health Care Arrangement (the "CIN OHCA") pursuant to the Health Insurance Portability and Accountability Act of 1996 ("HIPAA") because together the CIN participants constitute:

  • A clinically integrated care setting in which individuals typically receive health care from more than one health care provider; and/or
  • An organized system of health care in which the CIN participants hold themselves out to the public as participating in a joint arrangement and participate in one or more of the following joint activities:
    • Utilization review
    • Quality assessment and improvement activities
    • Payment activities
    • Clinical integration

The participants in the CIN OHCA have entered into a Declaration of Participation outlining the terms and conditions of participation in the CIN OHCA.

Suburban Health Organization, Inc. has the authority to enter into HIPAA Business Associate Agreements on behalf of the CIN OHCA.

This Declaration is effective beginning on the dates such activities began and shall remain effective until such activities cease or all CIN participants terminate their CIN participation agreement.
