During the 2025 Wellvana Accountable Care Symposium (April 15th-17th in Orlando, Florida), Leanne Schaller from Logansport Memorial Hospital was recognized as the 2025 Population Health Champion of the Year! The Wellvana team shared the following statement: “Leanne is a passionate leader in the ACO, demonstrating innovation and an unwavering dedication to improve the health and well-being of our patient population. Through data-driven strategies, she has implemented innovative care coordination initiatives that focus on proactive care and disease management, fostering collaboration across the full care team to provide a holistic experience for the patient. Employing patient education and targeted interventions, she has helped to reduce preventable hospitalizations, readmissions, and emergency department visits by addressing behavioral health needs, chronic disease management, and social determinants of health, working tirelessly to reduce disparities in care and access among underserved populations.” Having partnered with Leanne for a number of years, this award comes as no surprise.
Representatives from multiple SHO partner hospitals were in attendance. Many of these attendees gathered for a group picture at a networking event, providing everyone with a chance to meet in person and chat about the incredible work that is being done throughout the SHO network.
